LOW PLAN

  • In-Network Benefits

  • Low Plan Deductible: $3,500 individual/$7,000 family

  • Coinsurance: 20% after deductible is met.

  • Max out of pocket: $6,000 individual/$12,000 family

MID PLAN

  • In-Network Benefits

  • Mid Plan Deductible: $2,000 individual/$4,000 family

  • Coinsurance: 20% after deductible is met.

  • Max out of pocket: $5,000 individual/$10,000 family

HIGH PLAN

  • In-Network Benefits

  • High Plan Deductible: $250 individual/$500 family

  • Coinsurance: 10% after deductible is met.

  • Max out of pocket: $2,250 individual/$4,500 family

Specialty Pharmacy Services & Home Delivery Program

Specialty pharmacy services are provided by CVS Specialty. Home delivery is provided by CVS Caremark. Certain controlled substances and prescriptions that have expired will require a new prescription from a doctor.
